By placing this order, I confirm and agree to the following: 

Eligibility 

• I am an active frontline member of staff and eligible to order uniform items required for my role. 
• I understand that the cost of these items will be deducted from my weekly pay, either in full or in instalments, depending on the payment option I select. 

Accuracy of Information 

• I confirm that the personal details, size selections, and delivery information I have provided are accurate. 
• I accept responsibility for any delays or replacement costs resulting from incorrect information. 

Payment & Deductions 

• I agree that the total cost of the uniform items ordered will be deducted from my weekly pay. 
• I understand that I may choose for the total amount to be deducted either: 
• In one full deduction, or 
• In instalments, as outlined in the payment options provided. 
• I consent to these deductions being processed through payroll until the full balance is cleared. 

Usage & Condition 

• I acknowledge that although the uniform is purchased through deductions, I am required to maintain it in a professional condition. 
• I will only wear the uniform while on duty or when travelling to/from duty as required. 

Replacement of Lost/Damaged Items 

• I understand that any lost, misused, or excessively worn items are my responsibility and must be replaced at my own cost. 

Non-Transferable 

• I agree that all uniform items ordered are for my personal use only and must not be transferred, sold, or given to any other individual. 

Order Limit 

• I acknowledge that each order placed is subject to approval and must follow the company's uniform policy and deduction process.
